Hometown News Release

The Hometown News Release Program is an avenue for soldiers to share their accomplishments, i.e., awards, decorations, etc., with family members and communties back home.

Instructions

  1. Download and fill out the form DD 2266
  2. Return the filled out form to the Public Affairs Office by email, fax, or mail.

Social Security Number
This information is used solely to track the form through the system. It is not released as part of the story. Privacy Act Statement: Make sure you read and understand this statement. Signing and dating this form gives Hometown News the right to send the information on this form (except for the social security number) to your hometown newspapers.
Block 3:
Branch of Service
Self Explanatory.
Block 4:
Status
Self Explanatory.
Block 5:
Rank
This is your military rank: PFC, SPC, SGT, etc.
Block 6:
Pay Grade
This is where you put E-3 or O-2, etc.
Block 7:
Name
Self Explanatory.
Block 8:
Gender
M for Male and F for Female.
Block 9:
Event
Be as specific as you can. Attach additional files like fact sheets, cover releases and images to the DD 2266 as needed.
Block 10 - b. (1):
Next of Kin
Required to have both parents’ first and last names, their relationship to the soldier, along with a complete street address, city, state and complete ZIP code.
Blocks 11, 12, and 13
Self Explanatory.
Block 14:
Present Unit
Spell out unit, base and location.
Block 15:
MOS/AFSC
Don’t use number/spell out completely.
Block 16:
Present Job Title
Spell out job title if different from MOS/AFSC.
Blocks 17-20
Self Explanatory.
Block 21:
Signature
If submitting electronically just type your name in the box. This will tell us you have read and acknowledged the privacy act statement and you’re willing to have this information released to your hometown newspapers.
Block 22:
Date
Forms should be submitted within 60 days of the event, but it’s still a good idea to send these forms in as soon as they’re filled out.
